Custom Fields


Form Field Types

Action Form supports various field types, from simple text boxes to complex controls such as date pickers. The type determines how users interact with the form to submit data. 


Field types are specified in the Type drop down box of the field section on the Form Builder (Step 2: The Form) page. There are six field type categories; Open Questions, Closed Questions, Address, Hidden Fields, Static and Date and Time. A description of each category and their field types are specified in the bullet list below.

  • Open Questions
    Open Question field types are text fields that allows the user to enter a response to open ended questions. You can specify for example Email, Username, and Password text fields. For additional documentation on Open Question field types go to the Open Questions page.

  • Closed Questions
    Closed Question field types are predefined radio buttons and check boxes that allow the user to select an option. You can specify for example, multiple choice and true or false questions. For additional documentation on Closed Question field types go to the Closed Questions page.

  • Address
    Address field types are text fields for entering address related information, for example, country name and region code. For additional documentation on Address field types go to the Address page.

  • Hidden Fields
    Hidden field types are hidden fields that the user is unable to see on the action form. For additional documentation on Hidden field types go to the Hidden page.

  • Static
    Static field types does not include any fields on the action form. For additional documentation on Static field types go to the Static page.

  • Date and Time
    Date and Time field type allows the user to specify a date from a calendar. For additional documentation on the Date and Time field type go to the Date Field Type page.

  • Likert Scale
    Likert Scale field type provides the user with a series of responses based on if they disagree or agree, how often they perform a task or the importance of a task. For additional documentation on the Likert Scale go the Likert Scale page.

  • Multiple Choice
    Multiple Choice field type allows you to provide the user with a list of options using drop down menus, check boxes and radio buttons. For additional documentation on multiple choice types go to the Multiple Choice page.

Selecting Field Types

  1. To select a field type from the categories, click on the Type drop down box next to the Title field.

    • The Type drop down box is identified with the green arrow on Figure 1.1.

    • You will see the categories and the field types below each one.

  2. Then select the field type relevant to the text you entered for the Title.
